Can a Green Card holder lose their permanent residency status if they live outside the U.S. for an extended period? What are the rules?

Sure they can. You actually can't stay away from the US for too long or you risk losing it.

Yes, a green card holder can lose residency if they stay outside US for too long. If it is more than 6 months, you might have trouble coming back. After 1 year, they will think you abandoned your residency. But you can get re-entry permit to avoid this.
