My case hasn’t been touched in a long time and I’m starting to wonder at what point it’s reasonable to truly worry rather than just being impatient, especially when processing times vary so widely.

That’s a really common question and a hard line to judge, especially with how inconsistent timelines are. Most people don’t truly need to worry until their case is clearly outside normal processing times for their category and office, or unless something like an RFE or notice is overdue. Long periods of silence by themselves are unfortunately normal with USCIS, so until you’re well past posted timelines or your attorney flags a specific issue, it’s usually impatience and stress rather than a sign that something is wrong.

Waiting time have significantly been extended due to the backlogs. But you'll definitely get an update no matter how long it takes. Another thing you can do is speak to an immigration attorney for professional advice
