If my fiancé comes to America on tourist visa and we decide to marry, can we avoid the fiancé visa process and just adjust status?

It can work but it is risky. Immigration may see it as visa fraud and deny your fiance from adjustment of status

If you don't want any trouble, your fiance should just apply for K1 visa

Adjustment of status may be denied unless the original intent was truly to visit. K-1 visa is safer
