If your EAD has been lost, stolen, or destroyed, you must file a new Form I-765 with USCIS. Indicate that you're applying for a replacement card in the appropriate section of the form. Along with the application, you must submit the required filing fee, two recent passport-style photographs, and a copy of a government-issued photo ID such as a passport or driver's license. If available, include a copy of your prior EAD or a receipt notice for your previous Form I-765. It’s also a good idea to include a short explanation or sworn statement describing how the card was lost or stolen. The replacement EAD usually takes several months to process.

If your EAD gets lost or stolen, don’t panic. You can get a new one by filling out and submitting Form I-765 again. On the form, make sure to choose the option that says you’re replacing your card. You'll need to include some paperwork, like ID, a couple of passport photos, and the filing fee. If you still have a copy of the old card, include that too. It takes a few months, so apply as soon as you can.

USCIS permits individuals to request a replacement EAD by refiling Form I-765 and selecting the appropriate reason for replacement. The application should be accompanied by a valid government-issued ID, recent photographs, and supporting documentation such as a copy of the lost card or a previous receipt notice. Applicants must pay the required fee unless eligible for a fee waiver. A written statement about the loss or theft may be included, and in some cases, a police report can strengthen the application, though it is not required. USCIS will issue a replacement card if your request is properly filed.
