Hello I’m currently applying for a military advanced parole in place as a daca recipient. I recently got a letter stating that I needed to send uscis a written declaration explaining where when and how I last entered the United States. Has anyone here gone through the same thing or written one I don’t know the format or if it’s a specific form I just don’t want to mess it up. Thanks in advance!

I don't think there's a format, all you need to do is just write the declaration and be honest in your writing

When filing a DACA renewal, you must respond to the questions that ask about travel. You should attach copies of your advance parole document, entry stamp in your passport, and a printout of your Form I-94 that you can obtain online.
